Background

In the Democratic Socialist Republic of Sri Lanka (hereinafter, "Sri Lanka"), about 11% of the total population is 65 years or older, and it is predicted that the percentage will be over 21% by 2050 (Source: United Nations World Population Prospects 2022). Not only will the burden of illness due to non-communicable diseases (NCDs) and the prevalence of dementia increase, but also the number of elderly people living with disabilities increase, it is required to respond the both needs of the health care services and social services. Under these circumstances, the Cabinet decided on a national policy for aging in 2006, and a revised version is currently being formulated (as of September 2022). In January 2017 and elderly care is positioned as a national priority. This policy has 7 strategies such as the establishment of mechanisms to strengthen the health system for service provision, cross-sectional coordination, facilities, and human resources management for the provision of equal and comprehensive services and treatment, prevention, rehabilitation, and evidence-based investigation. However, while developing these policies, it turned out that elderly people do not receive sufficient care at both health care facilities and nursing homes and at home. In response to this issue, the Ministry of Health started measures such as the renovation of current health facilities into the intermediate care facilities. Besides, it is necessary to take measures to establish a service provision system of medical and social services for the elderly population.

The Overall Goal

The community-based health and social care model for elderly persons ("The model") is utilized for a wider application.

Project Purpose

The model is disseminated for the purpose of a wider application.

Output

  1. Planning and coordination mechanism for health and social services in the community is established in each pilot site.
  2. The situation of health and social services for elderly persons in each pilot site is analyzed by the mechanism established in the output 1.
  3. The models are developed at each pilot site for community-based health and social services for elderly persons.
  4. The coordination of health and social sectors at the central level and the coordination between central and local levels are improved.
  5. Recommendations are developed for a wider application of the models.

Project Activities

[Output 1]

1.1. Identify the relevant authorities and personnels in each pilot site.
1.2. Establish a working committee at each pilot site.
1.3. The working committees decide the roles and responsibilities of the participating authorities and personnels in the working committees.
1.4. The working committees formulate the plans for the activities of the working committees in each pilot site.

2.1. Prepare the questionnaires for collecting the data of elderly persons and their families as well as the data of health and social services.
2.2. Each working committee analyzes the situation of the elderly persons and their families in each pilot site.
2.3. Each working committee analyzes the situation of health services in hospitals and the community.
2.4. Each working committee analyzes the situation of social services in the community.
2.5. Each working committee identifies the mechanism of providing the health and social services for the elderly persons in each pilot site.
2.6. Each working committee identifies specific challenges of service provision for the elderly persons based on the analysis in each pilot site.

3.1. Each working committee identifies a few priority challenges in each pilot site.
3.2. Each working committee formulates action plans for solving the challenges they prioritize in each pilot site.
3.3. Based on the action plans, the health and social services for elderly persons are provided in each pilot site.
3.4. Each working committee monitors and assesses the implementation of the action plans and make necessary modifications.

4.1. Identify the relevant authorities of health and social service at central as well as provincial and regional levels for coordinating and monitoring the implementation of the Project.
4.2. Establish Project Implementation Committee (PIC) and Technical Working Groups (TWGs) for supporting the activities of the working committees in pilot sites.
4.3. PIC and TWGs regularly monitor and assesse the implementation of the Project.
4.4. PIC regularly gives the feedbacks to TWGs and the working committees in each pilot site.

[Output 5]

5.1. The Project (Sri Lankan side and Japanese side) draws the lessons and suggestions for a wider application of the models.
5.2. The Project (Sri Lankan side and Japanese side) documents the recommendations for a wider application of the models.
5.3. The Project (Sri Lankan side and Japanese side) submits the recommendations to the relevant organizations.
5.4. The Project (Sri Lankan side and Japanese side) holds the national and international seminars for the dissemination of the models and sharing the best practices.
