In the end, JOCV and their counterparts gave the lectures and led the group work on the usage of the International Classification of Functioning, Disability, and Health (ICF) for team approach in seamless care. During the group work, the participants were divided into 7 groups and did case study of IMC or LTC to make a treatment/care plan by using ICF. The case examples were a disabled child, an elderly person who is on IMC, and a person with Spinal Cord Injury (SCI) at the Industrial Rehabilitation Center. Through the discussion with multi-disciplinary, holistic treatment/care plans were created by the participants, including both medical and non-medical aspects.

The S-TOP team has positive expectations that the team approach introduced in this seminar will be utilized by the participants to create more effective and efficient IMC and Seamless service in the pilot sites.

The feature of this seminar was collaboration of JOCV and S-TOP Project, which utilized the advantages of each JOCV and S-TOP Project and actualized the learning from theory and application into practice for the daily works of the participants.
